Dear Ms. Smith,
Me and my husband are expecting a welcome addition to our family with the coming of our 3rd child sometime between October 10 to 21. As advised by my gynecologist, it is necessary for me to take a leave of absence and with this, I request your good office to please allow me to take a Parental leave from the period of October 1st up to the 7th of December 2009.
My request for 9 weeks of Parental Leave is necessary for me to take care of my newborn on his first few months of life. I will be able to resume my responsibilities in the office on the 8th of December but in case of unforeseen medical event, I will be applying for a leave extension.
I will endorse all necessary documents to my supervisor to facilitate smooth transition. I will also provide your office and my supervisor my contact details in case the need arises.
